QAPI Example and AI Response For context, my issue is: A resident had an order for a blood thinner that was changed to a different blood thinner. The nurse did not notice that the previous blood thinner was discontinued and accidentally gave the resident both the new and discontinued blood thinner simultaneously. My process improvement plan is as follows: The nurse discontinued medication was removed from the medication cart. All medication carts were audited to ensure no other discontinued medications were present for any residents in the facility. The nurse (and all nurses on staff) who discontinued the order was re-educated on the appropriate process to remove the discontinued medication and dispose of it at the time the medication is discontinued. The nurse (and all nurses on staff) who gave both doses of medication were re-educated on the appropriate standard of practice for administering medication. Daily for one month and then weekly for two months, all discharged medications are audited to ensure they're appropriately removed and disposed of when discontinued.
